Unable to activate 200mb Free Data Plan on Nexus 7 LTE

Pretty frustrated here.

I've spoken to 4 different people at T-Mobile and cannot get my tablet activated on the 200MBfree data plan. I'm using the SIM that I bought from T-Mobile directly to do just this.

 

Also when I try to create the account/active online via the tablet I'm getting "Your Mobile Internet Management sessions has timed out"
Does anyone have any idea what's going on here?
