How do I go about reporting fraud?

I have experienced what I believe to be misconduct with the a manager and the retail sales leader.  This involves a lease of a cell phone to an unauthorized person on the account and then a cover-up to try to hide the fraud that was committed, including the breach of privacy and security laws.  I first addressed the issue of the misconduct with the manager and she did not even bother to investigate the issue, instead she stated that no misconduct had occurred and that there was nothing she could do about it, and that she would contact me later in the day to tell me what happened in her investigation.  She never contacted me so I contacted the store again and spoke with the self-procalimed person in charge, and he assured me that everything was going to be okay and that he would look up the papers that were signed to make sure that everything was done by the book.  Several minutes passed and he returned, but informed me that he couldn't show me anything about the account because I wasn't the primary account holder and there were no authorized users on the account. (All of the authorized users had been "magically removed" after I spoke with the manager earlier in the day.  I was then stonewalled by all of these "Security" measures which prevented me from getting any information about the account, when just a few weeks prior an "unathorized" user was able to lease a phone! This is all a ridiculous ploy to cover up the misconduct. Please direct me in what I need to do to fix this and not allow this to happen again to myself, or any other T-mobile customer.
