ETF Reimbursement issue

I got it all approved and everything. But, I haven't gotten my reimbursement in the mail!?

I recently moved, during the process. I'm assuming it got sent to my old address. But, Sprint is now sending me to collections..because it's been so long. I don't know what else to do at this point.

If there's anything you can do to help, it would be great
